Your First Appointment

Please arrive 15 minutes before your scheduled appointment time in order to fill out the necessary forms. If your visit is to be covered by insurance, we will need to see your insurance card(s). Worker’s compensation claims also require authorization from your employer but not for the first free visit. Motor vehicle claims require insurance carrier name and claim number.

Please bring these items to the Team Doctors clinic on your first visit:

  • Current insurance information and insurance cards (or your spouse’s insurance card if applicable)
  • List of medications you are currently taking
  • Recent medical records, MRI/CT scans and/or x-rays from other physicians pertaining to the problem you want treated
  • Mention you found us on the web site and Dr Stoxen will consult with you at no charge for the first visit

 If you were involved in an auto accident, please bring these additional items with you:

  • The above items including your insurance card, spouse’s insurance card, and your auto insurance card. (We can bill your insurance for care resulting from an auto accident. Your heath insurance company can get reimbursed from the auto insurance carrier responsible for the payment).
  • The other party’s auto insurance information
  • The police report
  • Your attorney’s information (if applicable)
  • Hospital, emergency room records, x-rays, and results of diagnostic tests taken at a facility relating to the injuries sustained in the auto accident

If you were injured at work and want the treatments covered under worker’s compensation, we can do that if you bring the following items:  

  • The above items including your insurance card, spouse’s insurance card, your company’s workman’s comp insurance carrier name, and the adjuster handling your claim
  • Your attorney’s information (if applicable)
  • Hospital, emergency room records, x-rays, and results of diagnostic tests taken at a facility relating to the injuries sustained in the accident

Medicare – we do not participate with Medicare.

Medicaid – we do not participate with Medicaid.

Just 5 Minutes from Midway Airport – anyone can fly into Chicago, take a $5 taxi to our office and get great treatment before heading into the city for your sports event or business activities.

Just 20 mins from Downtown Chicago – If you are here on business, sports competition or a theaterfor the performing arts and wish to visit Dr Stoxen at TeamDoctors, we can arrange for your transportation.

The distance from Downtown Chicago is usually around 20 minutes. Your treatment will be well worth it!
From Downtown: Take Lake Shore Drive, south to Interstate 55, South, (travel 3 – 4 miles) to Pulaski Exit South, travel 2 miles to 6432 S, Astoria Plaza and Team Doctors

From Ohare Airport: Take the 190 to Interstate 294, Indiana Toll Road, South, (travel several miles) to I-55 North, (Travel 3 – 4 miles) to Pulaski South (travel 2 miles) to 6432 South Pulaski, Astoria Plaza and Team Doctor
(Approximately 35 minutes)

From Midway Airport: Take Cicero Avenue South to 63rd Street, Go East or Left one mile to Pulaski South, turn Right or South on Pulaski and go one block to 6432 S Pulaski, Astoria Plaza and Team Doctors
